How to File a Request in Case of Failure to Pay for Support

The process will entail filing an application form and stating what your complaint is. From there, you will receive an official who will look into your case to determine if it is worth taking action or not. The process is as follows:

Step 2: Read and sign it at the bottom.

Step 3: Access and download the application form.

Step 4: Fill in the form up to part V with the parents’ names and income details, children information and orders.

Step 5: Once done, send it to your local child support office.
